My Child Has An Allergy...

Food Allergy Research & Education

If your child has allergies that requires an epinephrine auto-injector or antihistamine to be treated, you must submit this FARE (Anaphylaxis Medication) Form along with the Individual Permission Form for Administering Medication when providing these medications to BLC.

If your child has allergies or another condition that requires an epinephrine auto-injector, an antihistamine, or another medication/healthcare procedure to be treated, you must submit this INDIVIDUAL PERMISSION FOR MEDICATION OR HEALTH CARE PROCEDURE when providing these medications to BLC.
